They came up with the idea of a young man with a quick wit and a dry humor as a bumbling medical student and they used their own personalities as the model for the character. Levine and Lampert noticed that the popular characters of the television show "Scrubs" seemed to have a similar voice to their own and they decided to create "J.D." as a contrast to those characters.

The character's first appearance was in "Scrubs" season 1 episode 3 "My House", where he was introduced as a new emergency room doctor at Sacred Heart Hospital. The character was well received and the writers started writing for J.D. more frequently, including his own story arcs and his own character development. Levine and Lampert were later credited as the creators of the character, with Lampert becoming the executive producer for the series.
