His father was the Roman general and leader Julius Caesar. He was the only son of Caesar's second marriage and the closest living relative of Caesarion's mother.

Ptolemy XV Caesar was named after his maternal grandfather. When he was born, his father was assassinated by members of the Roman senate who were angry at Caesar's civil war with Pompey. He was given the name Ptolemy XV in honor of his paternal grandfather. Ptolemy XV Caesar was very close to his mother. During his early childhood, he lived with his mother in Alexandria, Egypt. When he was three years old, he received a copy of William Shakespeare's play Julius Caesar from his father's troops in Rome. It is thought that this was a gift from his father to his newborn son.

He was the last pharaoh of ancient Egypt, reigning with his mother Cleopatra from 2 September 44 BC until her death by 12 August 30 BC, and as sole ruler until his death was ordered by Octavian, who would later become the first Roman emperor as Augustus.
