The Joker appears in the Joker Dilemma, a fictional work of a "psychologist" who tries to get a patient to become a psychopath. The main character, the Joker, is the psychopath, and his psychiatrist strives to save the main character (whom he calls The Joker) from himself by showing him his true nature. The Joker Dilemma ends with both characters "dying" (i.e., committing suicide), but in this case, the psychiatrist is right in that the main character is indeed a psychopath, the only question is the means by which he became one. The main character, in his last words, says that he has always wanted to be a psychopath; in other words, he was never interested in becoming one. The psychiatrist was unable to save him.

The Joker's backstory in "The Killing Joke" reveals that he was an escaped psychopathic patient of the psychologist who treated him for his schizophrenia. Batman takes advantage of this information to use The Joker in his schemes, but not before convincing the Joker that he is not insane and therefore does not need to be cured.
