A Ms. O was the former director of Precinct 13579 in the 1980's, back when Oprah and her partner O'Donahue were agents at the squad. As a test to see if Oprah was qualified to take her place as leader, she fabricated the identity of the Patternista: A villain who spoke in numerous sounds in certain orders; hence her name the Pattern-ista. Oprah passed the test and became Ms. O, and the former leader retired afterwards. However, in an alternate timeline, she fired Oprah after failing the test, with Odd Todd getting promoted instead.
