The name “Supreme Kai” is a reference to the Japanese title for the highest authority in something. In Japan, it’s usually a position of power, rather than a promotion to a higher rank. For example, in the Japanese government, there’s a cabinet minister for foreign affairs and a cabinet minister for internal affairs and there’s also a “foreign minister” who’s the head of the foreign ministry.
As a result of the silliness of the name and what it means in English, I’ve heard some people refer to Supreme Kai as an “imperial” having such a ridiculous name.
While I haven’t seen this for myself, it seems like people confuse the Japanese position of “Supreme Kai” with its English translation of “Kai”. As I understand it, the Supreme Kai isn’t an “emperor” and there is no “imperial family” (at least none that I’m aware of).
