If you look at the "Recommended by" comments, you can see that people are commenting on his personality, not his appearance. He's not scary, he's not confusing, he's just "creepy" and "odd" and "one of the best remixes of all time".
Nobody is saying that he's scary or confusing, only that he's weird. A fundamental difference between Nightmare Sans and other Freaky Undertale characters.
I think it's very possible he isn't trying to be scary. He's just... different. Different doesn't necessarily mean bad. There are characters in Undertale who are different but amazing. Trickster is one, Mettaton is another, and even the many characters with the same name (and the same personality type) are both very different and not at all bad.
But there is a difference between simply different and actually disturbing. I think that's the difference between Zesty Cream and Zesty Cream's brother.
If you look at Zesty Cream's personality, it is very... interesting.
Nightmare!Sans is the secondary antagonist of Underverse Season 1, and a master for Cross and X-Event!Chara. After Cross ends up having negative feelings, Nightmare enters his lonely universe, and they both create a deal that will allow them to extract parts of other worlds to his. He doesn't start causing problems until during Underverse 0.3, at which Nightmare start messing around with other people's emotions, and threatens to kill them.
